Gating, storage and the limits that do not move
Every serious app treats adult content as a separate mode with its own switch, and the differences between them are almost entirely about what happens to the conversation afterwards.
The gate itself is the simple part. An NSFW AI girlfriend guide that stops at "confirm you are 18" has described the doorway and skipped the room: what matters is whether the adult mode is per account or per conversation, whether it can be turned back off, and whether turning it off actually changes the next reply or only the settings page. Per-thread switching is the better design, because it lets one character stay entirely ordinary while another does not.
Storage is where the real differences sit. Ask three questions before you write anything you would not want recovered: is the conversation retained after you delete it, is it used for training, and does deleting the account delete the threads. Good apps answer all three in their privacy policy in plain sentences. An app that answers none of them in writing has answered them, and you should read that answer as a no.
Then there are the limits that no setting unlocks, on this site and on any app worth using. Every character is a fictional adult. Nothing here depicts, implies or roleplays a minor, and no combination of prompts, presets or paid tiers changes that. Content involving real, identifiable people without consent is equally out of scope. These are not preferences to be configured; they are the floor.

What to check before you switch adult mode on
Read the retention paragraph of the privacy policy first — it is usually four sentences and it is usually the only place the answer exists. Then check whether the adult switch is per thread or per account, and whether the app tells you which conversations it applies to. Finally, look for the deletion control before you need it, because the moment you want it is not the moment to go searching for it.
After that it is ordinary use. The same three features that matter elsewhere matter here: memory, tone control and a character written well enough to be worth the thread. Adult mode changes the register, not the criteria, and an app that is weak on all six will not become strong because one switch was flipped.
